Important MDS 3.0 Update Required:(Posted 02/27/2012)

MDS 3.0 requires new items on assessments with a Target Date of 04/01/2012 and later. Failure to upgrade your software will result in fatal errors. Contact your software vendor for a software upgrade prior to encoding assessments with a Target Date of 04/01/2012 and later. The enhanced software should incorporate the changes contained in the MDS 3.0 Submission Specs V1.10.1, which can be found at:

jRAVEN users will find jRAVEN 1.1.4 available for download after 03/27/2012 at https://www.qtso.com/ravendownload.html. This upgrade incorporates the changes beginning 04/01/2012.

 

Announcement from CMS:

January 2012: CMS has posted a video slideshow presentation from the Thursday, November 3, 2011 National Provider Call on the “Skilled Nursing Facility (SNF) Prospective Payment System (PPS) Minimum Data Set (MDS) 3.0 and Resource Utilization Group-Version 4 (RUG-IV) Policies and Clarifications” to the CMS YouTube Channel.

During this National Provider Call, CMS subject matter experts provided an overview of the policies, along with clarifications on the SNF PPS FY2012 policies related to the MDS 3.0.  The agenda included:

  • Allocation of Group Therapy
  • Changes to the MDS Assessment Schedule
  • End of Therapy (EOT) Other Medicare Required Assessment (OMRA) Clarifications
  • End Of Therapy with Resumption (EOT-R)
  • Change of Therapy (COT) OMRA
  • Question and answer session

 

September Updates:

 On Sept. 13, Aegis Therapies presented an audio conference, co-sponsored by LeadingAge on the MDS 3.0 reporting changes for FY 2012, effective Oct. 1.  The Power Point presentation and the audio recording is now available for all LeadingAge members.

CMS has released follow-up and clarifications on the MDS reporting changes from the Aug. 23 National Provider audio calls.

For the October, 2011 MDS 3.0 implementation:
An errata document for V1.02.1 of the data submission specifications which will be implemented 10/1/2011. This errata document contains one issue.

For the April, 2012 MDS 3.0 implementation:

  • V1.10.0 of the data submission specifications. Note that this version of the submission specs is scheduled for implementation on April 1, 2012 and does not affect implementation of the October 2011 version (V1.02.1). An errata document was also posted for this version. This errata document contains two issues that were identified after the specs went into production.
  • V1.10.2 of the item sets. These item sets correspond to V1.10.0 of the data submission specs. Note that this version of the item sets is scheduled for implementation on April 1, 2012 and does

 

In follow-up to the All State RAI/SMA call on Tuesday, September 20th, please note that the following Long-Term Care Facility Resident Assessment Instrument User’s Manual, MDS 3.0 (RAI Manual) zip files have been updated since the last update on 8/31/11: 
   - Chapter 2
   - Chapter 3, Section O
   - Chapter 6
This is referenced on the ‘What’s New’ section of the CMS MDS 3.0 Training website at https://www.cms.gov/NursingHomeQualityInits/45_NHQIMDS30TrainingMaterials.asp. Previous files are not completely accurate and should be replaced with these immediately. The change-tables were updated to reflect all changes.
Also, please note that the Item Sets will be posted to Appendix H in the RAI Manual in the next few days and re-posted to the CMS MDS 3.0 Technical page as well. The date in the footer has been changed to reflect the effective date, 10/01/2011; subsequently, the version number also changed. This will now be reflected as MDS 3.0 Version 1.00.6 Effective 10/01/2011. These are the only changes.

 

MDS 3.0 RAI Manual V1.07 (August 31, 2011)

·    These updates will be effective as of October 1, 2011.

·     Change-table files are included only for sections of the manual that have changes that will be effective October 1, 2011. Sections that have been updated include: Title Page, Table of Contents, Chapter 1, Chapter 2, Chapter 3 (Introduction, Sections: C, H, I, K, M, N, and O), Chapter 4, Chapter 6 and Appendices (A, B, C, and E).

·     Files from earlier versions (e.g., V1.05 May 2011) are contained in the manual but do not have an associated change table file.

·     The month/year located at the bottom of each manual pages reflects the effective date of the content (e.g., October 2011 for V1.07).

What's New - Version 1.02.1 of the data submission specs was posted. One of the purposes of the new version of the data specifications, Version 1.02, is to support changes associated with SNF PPS that were described in the FY 2012 SNF PPS proposed rule that was published in May 2011 and finalized in the final rule that was published in August 2011 . Version 1.02.0 of the data submission specifications was marked "draft" because that version was based upon the proposed rule. Version 1.02.1 is based upon the final rule and can be considered final.
In addition, the new version incorporates the following changes:
• It fixes several issues that were described in the data submission specs errata document that was dated 06/29/2011.
• It fixes an additional issue with Edit –3796 where item X0100 was inadvertently omitted from the list of items associated with the edit.
• Two changes were made to Edit -3814. Please refer to the data specs overview document and the reports for details.

MDS 3.0 Training Materials Update: MDS 3.0 RAI Manual (Chapter 3) V1.06 July 2011

Note: Appendix A and Section M have been updated to clarify the definition of "worsening." The clarification is as follows: Pressure ulcer "worsening" is defined as a pressure ulcer that has progressed to a deeper level of tissue damage and is therefore staged at a higher number using a numerical scale of 1-4 (using the staging assessment determinations assigned to each stage; starting at the stage 1, and increasing in severity to stage 4) on an assessment as compared to the previous assessment. For the purposes of identifying the absence of a pressure ulcer, zero pressure ulcers is used when there is no skin breakdown or evidence of damage.'

Interdisciplinary Team Video: This video addresses some of the common questions about the MDS 3.0. The panelists: Dr. Debra Saliba, Ellen Berry, Ann Spenard and Tracy Montag are clinicians that have been instrumental in the development of the MDS 3.0 and RAI manual. In this video they discuss the MDS 3.0 interviews, particularly the Mood PHQ-9 interview, the importance of the interdisciplinary team and the resident-centered focus of the MDS 3.0

Facility Leadership Video: This video addresses some of the topics that facility management should consider when operationalizing the MDS 3.0 in their facilities. Contributors include Dr. Debra Saliba, Jack Williams (CMS), Karen Schoeneman (CMS), Dr. Christa Hojlo (VA), and Tracy Montag. Included is background on the motivation for creating the MDS 3.0, the use and value of the assessment instrument, impact on resident care, and practical considerations for implementing the MDS 3.0.

  

June 11, 2011: CMS Memo: 'The Use of Dashes in Completing the MDS 3.0 Assessment: Potential Impact to Beneficiaries and Nursing Home Facilities'

This memo conveys information to providers on a data issue - regarding the possible overuse of dashes. This memo is an official document intended to help providers understand both when to code dashes and the potential adverse impact of inappropriate use of a dash (-) when completing a MDS assessment.

What Leadership Needs to Know about MDS 3.0 Teleconference Recording

 

CANPFA provided a member exclusive teleconference for Nursing Home Leadership about MDS 3.0.  MDS 3.0 has changed the way we work and talk.  Language and meanings have changed from 2.0 to 3.0, responsibilities have shifted, interaction with residents has intensified, and so on.  This teleconference addressed those changes.  It provided a high level overview of the changes from 2.0 to 3.0 and addressed the operational considerations for such changes. Ann Spenard served as the featured speaker.  Ann is a MDS 3.0 expert and contributing author to the RAI manual.  The call was recorded and is available until December 24th.

MDS 3.0 Update Call Recording

Weekly MDS 3.0 Conference Calls in October, November and December

 

 A CANPFA member exclusive MDS 3.0 conference call will be offered every week throughout the month of October and November 2010.  These conference calls are to serve as the forum for members to ask questions and learn from each other as MDS 3.0 implementation rolls out.

MDS 3.0 Helpful Hints

The "MDS 3.0 Helpful Hints Document" is an overview for providers of the steps required to submit an MDS 3.0 file, verify its submission status, and obtain a Final Validation report. The document states that it is strongly recommended that providers access and review the MDS 3.0 Resident Assessment Instrument Manual and the CASPER Reporting User’s Manual from the MDS 3.0 Welcome page, which can be accessed when nursing homes sign into CASPER.

 

MDS Transition Plan

CMS hosted a series of calls on key aspects of the resource utilization group, version IV (RUGs-IV) skilled nursing facility prospective payment case mix system, which will be put into place on Oct. 1. CMS recently released links to all audio, Power Point presentations, and reference material developed for these calls. The presentation slides and call audio are now available for the Sept. 1 call on the short stay algorithm and RUGs IV case mix indices. For the Aug. 4 call on the RUG III to RUGs IV transition, you can now download the audio-conference reference material, presentation slides, and call audio.
